Job Title: Living Well Coordinator (Social Prescriber)

Responsible to: Adapt (NE) and West Northumberland Primary Care Network

Hours: 37 hours per week, Monday to Friday 9.00am – 5.00pm. Two full-time posts (open to job share)

Salary: £27,595.92 per annum

Benefits: 25 days annual leave per year, plus statutory holidays (pro rata)

Location: The Living Well Coordination Service covers the 12 GP practices in the West Northumberland Primary Care Network (PCN)

We are recruiting for two full-time Living Well Coordinators. This is an opportunity to be involved in delivering and supporting solutions for primary care patients across the West Northumberland PCN.

Each Living Well Coordinator will be the named worker for 1-3 GP practices and will split their working time across these practices, as well as working from Adapt (NE)’s office in Hexham and home-working where appropriate. They will also be required to work in other places throughout the North East, as identified in your individual work programme.

The West Northumberland PCN practices are Bellingham Practice, Branch End Surgery (Stocksfield), Burn Brae Medical Group (Hexham), Corbridge Medical Group, Haltwhistle Medical Group, Humshaugh and Wark Medical Group, Prudhoe Medical Group, Scots Gap Medical Group (Morpeth), Sele Medical Practice (Hexham) and White Medical Group (Ponteland). Adderlane Surgery (Prudhoe) and Riversdale Surgery (Prudhoe) are also supported by the service.

Clients may live rurally beyond these areas.

We are recruiting Team Leaders for the West Northumberland Primary Care Network Living Well Service and the Northumbria Primary Care Network Social Prescribing Service.

Job Title: Team Leader

Location: Ponteland surgery and office base Cramlington with travel across the Northumbria PCN or Prudhoe surgery and office base Hexham with travel across the West Northumberland PCN

This is an opportunity to be involved in delivering and supporting solutions for primary care patients across the two PCN areas. We are recruiting Team Leader posts for each PCN area.

The Team Leader will work with a small team and be the first point of contact for staff. The Team Leader will report directly to the Adapt (NE) Director and liaise with identified staff within the PCN and when required individual Practice Managers and/or practice based Care Navigators across a range of GP practices.

You will be responsible for developing the service identifying opportunities to promote the service to potential patients, developing in partnership a robust plan to support community involvement.

We are looking for a part-time Driver to join our transport team, based in Hexham. The role will be to work on various contracted and Dial-a-Ride routes. You will need a D1 on your licence and have a flexible approach and friendly manner. We have schools contracts, Dial-a-Ride services across the area and lots of other varied and interesting jobs for our drivers here at Adapt (NE). Hours will be contracted with additional hours available.

Training on transporting people with varying abilities will be provided and you will also complete a MIDAS certificate.

The Northumbria PCN GP practices are Allendale/Haydon Bridge, Cramlington, Elsdon, Ponteland and The Rothbury Practice. You will be working primarily out of GP surgeries as well as, where appropriate, working from the Adapt (NE) office, visiting clients in their homes and meeting clients in other community settings.

Job Purpose

Social prescribing in the Northumbria PCN empowers people to take control of their health and wellbeing through referral to Social Prescribers who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support.

As a social prescriber, you will support individuals referred to the service by working in partnership with them to assess their non-medical needs and priorities and link them to suitable resources, groups and services. You will empower clients to access the support they need, which may include accompanying clients to local sources of help, where appropriate.

You will proactively build relationships with clients, offer support and develop collaborative, co-produced solutions to address their issues. You will develop relationships with community organisations and statutory services to maintain up to date knowledge and expertise on local services.

Overall purpose

This role is to support people with severe mental illness (SMI) to enable them to engage with primary care services and other initiatives which improve their physical wellbeing. The SMI Outreach Worker will reach out to people on the SMI register of the West Northumberland Primary Care Network to bridge the gap, and provide support which helps them to engage with physical health checks and follow-up interventions. The outreach worker will also encourage clients to develop more skills and knowledge around healthy lifestyles. As a partner in the Northumberland VCSE Alliance, Adapt (NE) is delighted to be part of this important work.

Overall purpose

RPR role

A Relevant Person’s Representative (RPR) is a person who is appointed to act on behalf of, and in the best interests of, somebody who is subject to a Deprivation of Liberty Safeguards (DoLS) authorisation. The person subject to the DoLS is referred to as the ‘relevant person’.

Most RPR work is based upon people living in a care or nursing home setting and will include working with people of all ages and from all backgrounds, including those with learning disabilities, people with mental health needs, people with physical and sensory impairment and older people.

The post holder will work in conjunction with other Adapt (NE) staff and will assist in the co-ordination and development of the service where appropriate.
