Job Title: Living Well Coordinator (Social Prescriber)

Responsible to: Adapt (NE) and West Northumberland Primary Care Network

Hours: 37 hours per week, Monday to Friday 9.00am – 5.00pm. Two full-time posts (open to job share)

Salary: £27,595.92 per annum

Benefits: 25 days annual leave per year, plus statutory holidays (pro rata)

Location: The Living Well Coordination Service covers the 12 GP practices in the West Northumberland Primary Care Network (PCN)

We are recruiting for two full-time Living Well Coordinators. This is an opportunity to be involved in delivering and supporting solutions for primary care patients across the West Northumberland PCN.

Each Living Well Coordinator will be the named worker for 1-3 GP practices and will split their working time across these practices, as well as working from Adapt (NE)’s office in Hexham and home-working where appropriate. They will also be required to work in other places throughout the North East, as identified in your individual work programme.

The West Northumberland PCN practices are Bellingham Practice, Branch End Surgery (Stocksfield), Burn Brae Medical Group (Hexham), Corbridge Medical Group, Haltwhistle Medical Group, Humshaugh and Wark Medical Group, Prudhoe Medical Group, Scots Gap Medical Group (Morpeth), Sele Medical Practice (Hexham) and White Medical Group (Ponteland). Adderlane Surgery (Prudhoe) and Riversdale Surgery (Prudhoe) are also supported by the service.

Clients may live rurally beyond these areas.

The service is looking for Living Well Coordinators to be named workers for referrals from GP practices in Hexham, Prudhoe and Stocksfield.

The successful applicants will also receive referrals from other West Northumberland practices in order to meet the needs of the service.

Each Living Well Coordinator will be the named worker for 1-3 GP practices and will split their working time across these practices, as well as home-working and/or working from Adapt’s office in Hexham where appropriate.

Initially these posts may be predominantly remote-working due to COVID-19.

Hours: Part-time – minimum of 22 hours per week, with the possibility of up to 29.6 hours per week being offered. Days and hours of work to be agreed with the successful applicant.

Salary:  £24,157 per annum (pro-rata)

Benefits:  25 days holiday per year, plus statutory holidays pro rata

Location: The Living Well Service covers the 14 GP practices in West Northumberland.

The successful applicant to this post will be the named worker for Burn Brae Medical Group in Hexham and will also be allocated referrals from other practices when needed.

Please note that you are unable to be the named worker for a practice you are registered at as a patient.
