We are recruiting for a full-time Link Worker. This is an opportunity to be involved in delivering and supporting solutions for primary care patients across the West Northumberland Primary Care Network (PCN).

Job Title:                                Link Worker – Living Well Coordination Service

Responsible to:                     Adapt (NE) and the West Northumberland PCN

Hours:                                     37 hours per week, Monday to Friday 9am – 5pm

Location:                               Office-based at Adapt (NE), Burn Lane, Hexham, NE46 3HN

Salary:                                   £22,549 per annum

If you have answered ‘yes’ to the above questions, then the Living Well Coordination Service would love to hear from you.

Our Link Worker will be the initial contact point for the Living Well Coordinating Service (social prescribing), providing administrative, triaging and signposting support to people when they are first referred to the service, as well as holding a small caseload of patients with lower level support needs. The successful applicant will be provided with full training and induction, including NHS social prescribing training.

This is a new role within the team and will be based at the accessible Adapt (NE) site in Hexham, which also houses Healthwatch Northumberland, Northumberland Independent Advocacy Service, transport services and the Adapt (NE)Community Café, Cafe @ Burn Lane.
